The form asks for your name and mobile number, and optionally your preferred service, consultation preference, area or PIN code (only for home visit requests), preferred date and time, and a short message about your concern.

Where it goes

The form currently works inside your browser only: when you submit it, a confirmation message is shown on the page. The details are not sent to a server and are not stored by this website. To actually reach the medical team, please continue on WhatsApp at +91 78876 10235 or call the same number. Anything you share on WhatsApp or by phone is handled by Dr. Zaid Khan and the medical team, and by the phone or messaging service you use.

Why it is used

The information is used only to understand your request and arrange a consultation. It is not used for advertising and is not shared for marketing purposes.

Please do not send emergency medical information through this website. For urgent or severe symptoms, contact local emergency services immediately.
